Project Description

The patient safety assessment program is more than just a survey, it is a process. The results of the survey are analyzed, site visits are conducted, and final reports are completed with recommendations for improvement.

To measure the culture of safety among our facilities, Baylor Scott & White Health its own assessment tool, the Attitudes and Practices of Patient Safety Survey, in 2003. In order to measure various degrees of patient safety the survey is divided into four domains.

Leadership
The degree to which hospital leadership promotes patient safety through their direct involvement and their ability to deal with staff issues that impact patient safety.

Resources
The degree to which the resources provided to staff are adequate to support a safe environment to give and receive care.

Teamwork
The degree in which there is teamwork among individual staff, departments and different professional groups.

Reporting & Feedback
The degree in which there is a safe environment that encourages speaking up to protect patient safety and to learn from near misses.

Patient Safety Assessment Program

The final report provides improvement opportunities and suggests initiatives to implement in order to improve patient safety. Along with improvement initiatives, materials and references are provided in order to assist with implementation of these recommendations.
