Vice President of the Chronic Care Continuum, Baylor Scott & White Health

Dr. Mae Centeno serves as Vice President of the Chronic Care Continuum for Baylor Scott & White Health (BSWH). She is also an adjunct faculty member at Baylor University College of Nursing and the University of Texas at Arlington. In her previous role at BSWH, Dr. Centeno served as Program Manager and Clinical Nurse Specialist for the Heart Failure Program and Advanced Lung Disease Center at Baylor University Medical Center. She frequently gives international presentations on topics such as reducing readmission rates and transforming care for patients with chronic medical conditions. She is currently co-leading a grant-funded project focused on risk stratification and transitional care.

Dr. Centeno earned a Bachelor Science in Nursing from Saint Louis University in the Philippines and a Master of Science in Adult Health from Texas Woman’s University. She earned her Doctor of Nursing Practice from Texas Christian University. She is a member of the American Association of Critical Care Nurses and is an inaugural member of the American Association of Heart Failure Nurses. Dr. Centeno is also a member of the National Quality Forum sub-committee on the All Condition Admissions and Readmissions Workgroup.
