Baylor Scott & White Health’s Patient Safety Program Now Available for External Organizations

The Patient Safety Assessment Program developed by Baylor Scott & White Health (BSWH) is now available for external health care organizations who are interested in enhancing their culture of safe care. This program comes from The Office of Patient Safety at BSWH, which has a long history of successfully improving the quality of care that patients receive and even is credited with creating a safer environment for health care employees. The Office of Patient Safety was formed in 2005 at Baylor Health Care System (BHCS), now part of BSWH, to address the increasing concern of patient safety across the health care industry. The mission driving the patient safety program at BSWH is to:

  • Achieve no preventable deaths;
  • Ensure no preventable injuries; and
  • Seek no preventable risk.

To measure the culture of safety among their facilities, Baylor Health Care System developed its own assessment tool, the Attitudes and Practices of Patient Safety Survey, in 2003. This survey is part of the patient safety assessment program called “A Culture of Patient Safety.” This program is now available for external health care organizations that are interested in improving patient safety at their facilities.

 The patient safety assessment program includes tools such as:

  • Patient Safety Culture Survey with Physician and Operating Room Add-Ons
  • Scoring Algorithms to Interpret Survey Results
  • Site Visit Framework
  • Recommendation Guide
  • Tools for Improvement (Methods, Policies, Worksheets, Checklists, etc.)
  • Implementation Materials (Marketing/Education Materials)

The patient safety assessment program enhances a culture of safe care. Please watch the following videos if you are interested in learning more about a culture of patient safety. If your organization is interested in implementing this survey process or would like to learn more, please contact Nanette Myers at or 214-265-3614.
